Annual Report

Due Dates

2020 Annual Reports for Community Water Systems (CWS) outside the Active Management Areas are due on or before June 1, 2021.

File your Annual Report

Please select one of the following options below to submit your Annual Water Use Report:

To report online, please use the newly enhanced reporting tool:  CWS Annual Report Online Reporting Tool. 

To report as a hardcopy, please contact the Community Water System Program at 602-771-8585 or [email protected] to request the forms.  

If your water system has a Designation of Adequate Water Supply (DADE), a DADE annual report must be filed as well. This form is available as a PDF document. Please include the completed DADE annual report with your CWS annual report.

If you have a Designation of Adequate Water Supply, the online reporting system will automatically redirect you to file the Designation of Adequate Water Supply annual report after you finish filing the Community Water System annual report.

To determine if you have a Designation of Adequate Water Supply, you can view the List of Designated Providers. Only providers on this list have a Designation of Assured or Adequate Water Supply.

Annual report GUIDELINES

How to access previous Annual Reports - This document will guide you through the process of downloading your previously submitted Annual Water Use Reports and additional correspondence related to your Community Water System.

Water Estimation Tool - Assists non-metered community water systems with estimating their annual water use.

Estimating water use based on energy records - These documents detail a number of suggested methods for estimating water use.  They all involve knowing the energy consumption of the well, possibly in conjunction with discharge information (such as pipe or channel flow in gallons per minute), or only information concerning the energy use if discharge information is unavailable.  These methods include estimating pumpage based on:

  1. Pipe flow and discharge information: Using electrical/natural gas energy records
  2. Open channel flow and discharge information: Using electrical/natural gas energy records
  3. Calculating pumpage based on using hour meters
  4. Estimating pumpage based on energy records
